An opinion piece by International Livestock Research Institute veterinary epidemiologist Delia Grace shines the spotlight on the global challenge of antimicrobial resistance and the need to tackle the problem while finding a balance between low access to antimicrobials (particularly in developing countries) and overuse of antibiotics in animal agriculture.

She writes:

“Antimicrobial use is a matter of access versus excess. Somehow, we must reduce the use of antimicrobial drugs in animals to tackle growing levels of drug resistance while ensuring that these life- and livelihood-saving treatments reach those who really need them.”

This week’s (4-8 September 2016) first joint international conference of the Association of Institutions for Tropical Veterinary Medicine and the Society of Tropical Veterinary Medicine featured a presentation on results on a survey of Taenia solium cysticercosis risk factors, perceptions and practices in smallholder pig production systems in Uganda.

Scientists from the National Livestock Research Resources Institute (Uganda), Makerere University and the International Livestock Research Institute conducted the survey in 1096 households in the rural and urban pig production systems in districts of Masaka, Mukono and Kamuli.

The study found that 63% of interviewed farmers were aware about taeniosis but less than 1% were able to make the link between taeniosis and cysticercosis in people and pigs.

Most farmers (94%) dewormed their pigs and 55% had clean water near the latrines designated for washing hands. Of these, 41.9% used water with soap to wash hands after latrine use.

The findings of the study point to a need to raise awareness among the pig farmers on the transmission cycle of pig-borne diseases like taeniosis and Taenia solium cysticercosis to help improve smallholder pig production and management.

A special issue of the African Journal of Food, Agriculture, Nutrition and Development (AJFAND) published in July 2016 and sponsored by the International Livestock Research Institute (ILRI) features 12 peer-reviewed scientific articles on aflatoxins in eastern Africa.

The three broad objectives of the special issue are to understand the health consequences of aflatoxins, characterize the extent of the problem and identify key elements to underpin the way forward to mitigation.

The holistic concept of ‘One World-One Health’ in disease prevention and control will be among the topics of discussion at the first joint international conference of the Association of Institutions for Tropical Veterinary Medicine (AITVM) and the Society of Tropical Veterinary Medicine (STVM) which is scheduled to place on 4-8 September 2016 in Berlin, Germany.

AITVM is a foundation of 24 veterinary faculties and livestock institutes based in Africa, Asia and Europe with the mandate to improve human health and quality of life by means of increased and safe food production in tropical regions through enhancement of research, training and education in veterinary medicine and livestock production within the framework of sustainable development.

STVM is made up of scientists, veterinarians and students from more than 40 countries with common interests in tropical veterinary medicine. It is a non-profit organization whose purpose is the advancement of tropical veterinary medicine, hygiene and related disciplines.

The joint conference will bring together animal health and production experts, senior and junior career researchers and students from all over the world to discuss research and development topics including animal and zoonotic disease control, food safety, genetic resources and biodiversity, rural development and animal production, training and capacity building and animal welfare.

The conference is organized by the Institute for Parasitology and Tropical Veterinary Medicine and the Food and Agriculture Organization of the United Nations Reference Centre for Veterinary Public Health of the Freie Universität Berlin.

The co-organizing institutions are:

The Lancet yesterday (18 Nov 2015) published a new series titled Antimicrobials: sustainable access and effectiveness in recognition of the World Health Organization’s inaugural World Antibiotic Awareness Week, 16 to 22 November 2015.

The theme of the global campaign, Antibiotics: Handle with Care, reflects the overarching message that antibiotics are a precious resource and should be preserved. They should be used to treat bacterial infections, only when prescribed by a certified health professional. Antibiotics should never be shared and the full course of treatment should be completed – not saved for the future.

The five papers in the Lancet Series cover access to effective antimicrobials as a global challenge, understanding the mechanisms and the drivers of antimicrobial resistance, maximizing access to achieve appropriate human antimicrobial use in low- and middle-income countries, exploring the evidence base for national and regional policy interventions to combat resistance, and international cooperation to improve access to and sustain effectiveness of antimicrobials.

Accompanying the papers are three comments that discuss achieving the balance between sustainable access and sustainable effectiveness of antibiotics, animal production and antimicrobial resistance in the clinic and national action for global gains in tackling the challenge of antimicrobial resistance.

Timothy Robinson, a scientist at the International Livestock Research Institute (ILRI), is the lead author of the comment titled Animal production and antimicrobial resistance in the clinic. His co-authors are Heiman F.L. Wertheim, Manish Kakkar, Samuel Kariuki, Dengpan Bu and Lance B. Price.

Antimicrobial resistance occurs when microorganisms (bacteria, viruses, fungi and parasites) develop the ability to continue growing in the presence of an antibacterial, antiviral, antifungal or antiparasitic substance to which they were previously sensitive.

The use of antibiotic drugs to prevent and treat livestock diseases is a key driver for the development of agriculture-related antimicrobial resistance which is now a global public health problem as antibiotics in food animals can enter the food chain and affect the health of consumers and communities.

In developing countries, antimicrobial resistant pathogens are commonly found in animals, animal food products and agro-food environments. However, the lack of national surveillance systems means that we do not have reliable estimates of the true burden of antimicrobial resistant infections in developing countries.

In addition to lack of accurate information on antibiotic use in developing countries, there is limited understanding of the sources of antimicrobial resistance in animal agriculture and the relative importance of different sources.

In order to address these concerns, the International Livestock Research Institute (ILRI) has this month (June 2015) produced a report which aims to identify key evidence gaps in our knowledge of livestock- and fisheries-linked antimicrobial resistance in the developing world, and to document ongoing or planned research initiatives on this topic by key stakeholders.

ILRI veterinary epidemiologist Delia Grace wrote the report which reviews the knowns and unknowns of

  • the prevalence of antimicrobial resistant infections in livestock and fish systems and products;
  • the health and economic impacts of livestock- and fisheries-linked antimicrobial resistance in the developing world;
  • technical capacity in developing countries to assess antibiotic use and antibiotic resistance in the livestock and fisheries sub-sectors;
  • key drivers of antimicrobial resistance in livestock and fisheries production in the developing world; and
  • modalities of reducing antibiotic use and levels of resistance.

She concludes with a call to address the global problem of antimicrobial resistance through an evidence-based approach which includes filling knowledge gaps, careful piloting of interventions and rigorous evaluation of success and failure.

The report was produced by ILRI for Evidence on Demand with the assistance of the UK Department for International Development contracted through the Climate, Environment, Infrastructure and Livelihoods Professional Evidence and Applied Knowledge Services programme, jointly managed by DAI (which incorporates HTSPE Limited) and IMC Worldwide Limited.

Livestock production is beset by many challenges. Animal diseases, in particular, can lead to reduced production of milk, meat and eggs. In some cases, animal disease infections may result in death, leading to loss of livelihoods for livestock keepers.

Livestock in developing countries suffer a high burden of preventable disease, and this burden is likely to increase as livestock systems become more intensive. Antibiotics and other antimicrobial drugs are often used to treat animal disease infections.

However, authors of a study published in March 2015 in the Proceedings of the National Academy of Sciences of the United States of America (PNAS) warn that the use of antimicrobial drugs in livestock production could contribute to the spread of drug-resistant pathogens in both livestock and humans, posing a significant public health threat.

The study also projects that by the year 2030, global antimicrobial consumption will rise by 67% and nearly double in Brazil, China, India, Russia and South Africa.

In light of the global nature of the problem of antimicrobial resistance, the World Veterinary Association and the World Medical Association prioritized the topic during one of the sessions at their global conference on One Health held in Madrid, Spain on 21-22 May 2015. The conference was organized in collaboration with the Spanish medical and veterinary associations.

Over 300 delegates from 40 countries attended the conference which was aimed at achieving stronger collaboration between physicians, veterinarians and all relevant stakeholders to improve various aspects of the health and welfare of people, animals and the environment.

Delia Grace, veterinary epidemiologist and leader of the Food Safety and Zoonoses program of the International Livestock Research Institute (ILRI), attended the conference and presented on antimicrobial use in developing countries, highlighting some results from research by ILRI.

The other conference sessions were on zoonotic diseases, natural disaster management, One Health in food production, and veterinary education of the One Health concept. More information is available in the conference report (PDF).

In April 2010, Miyazaki prefecture in Southwest Japan experienced an outbreak of foot-and-mouth disease, one of the most contagious animal diseases. Although the outbreak was successfully contained in just four months, by July 2010 there were 292 cases and 300,000 cows and pigs had been slaughtered. This resulted in an economic loss of about 2 billion US dollars.

In addition to the economic impact of the epidemic, the mental and psychosocial well-being of individuals and the community at large was also affected. For example, the sudden death of large numbers of animals caused considerable mental stress among farmers as well as the veterinarians and municipal government teams involved in the slaughter and disposal of infected cattle and pigs.

Restrictions on movement were imposed as part of efforts to prevent the disease from spreading; this led to stress-related symptoms among some residents, particularly the elderly. In addition, many farmers experienced depression and anxiety about the future following the loss of their livelihoods.

Recognizing the multiple impacts of the disease epidemic, a coordinated multisectoral approach was adopted, under the One Health concept, to tackle the disease as well as manage the mental health and psychological well-being of the residents of Miyazaki.

In a video titled Responding to an animal disease epidemic: Lessons from Miyazaki, various stakeholders who were involved in responding to the epidemic reflect on the usefulness of a One Health approach in helping to successfully respond to and overcome the challenges of the disease outbreak.

Featured in the video is Kohei Makita, an associate professor of veterinary epidemiology at Rakuno Gakuen University who is on a joint appointment at the International Livestock Research Institute (ILRI). Makita and colleagues had earlier published work on the collaborative response of veterinary and psychiatry experts to the 2010 foot-and-mouth disease outbreak.

As reported last week in a scientific paper published in the Proceedings of the National Academy of Science (PNAS), Global trends in antimicrobial use in food animals, worldwide antimicrobial consumption is expected to rise by a staggering 67% between 2010 and 2030.

Five countries—Brazil, Russia, India, China and South Africa (the so-called ‘BRICS’)—will experience a growth of 99% in antibiotic consumption.

Use of such drugs has grown as livestock systems intensify around the world to meet a growing world demand for meat, milk and eggs, particularly in developing countries.
